A Clinical Study on Acute Generalized Peritonitis


Abstract
This report is a clinical review of 212 patients with acute generalized peritonitis who were treated at the Dep:rtnent of Surgery of Cho Sun University Hospital from January 1979 to October 1982.
The results obtained are as follows;
1) The main causes of acute generalized peritonitis were peptic ulcer perforation, traumatic hollow viscus perforation, traumatic hemoperitoneum, perforated appendicitis and typhoid ulcer perforation.
2) The male to female sex ratio was 2.9:1.
3) The majority of patient with peptic ulcer perforation were 4th to 5th decade; with typhoid ulcer perforation and traumatic peritonitis were 3rd to 4th decade; with perforated appendicitis were 2nd to 3rd decade.
4) There were no seasonal differences in acute generalized peritonitis but typhoid ulcer perforation showed high incidence in Summer and Autumn.
5) About 61. 5% of patient *ith acute generalized peritonitis visited our hospital within 24 hours from onset but 6.3% of patient over 72 hours.
6) The incidence of subphrenic free air shadow in the plain X-ray was found in 87% of .cases in peptic ulcer perforation, 33.3% in typhoid ulcer perforation. 24.4% in traumatic hollow viscus perforation.
7) The most common post-operative complication was wound infection, followed by pulmonary complication, post-operative adhesive ileus and abdominal abscess in that order. Perforated appendicitis and typhoid ulcer perforation showed the highest incidence of wound infection.
8) The overall mortality of this series was 5.2%.
9) The cause of death was sepsis, hemorrhagic shock and acute renal failure.
